In the late 19th century sex workers actually called it a "below job." Over the years it eventually became "blow job."

However, it's more plausible than many folkloric etymologies. Such as the bogus "the word f*ck comes from a time when legally licensed bordellos had posted "Fornication Under Consent of King"'. Bogus because the cognate in German is ficken. You don't get cognates from acronyms!

It depicts a man walking down the hall of a hotel past the door of a room above which the transom is open. He overhears from inside the room: "I don't care what it's called, dammit--SUCK!"
